Previously you have told us that you have an impairment or condition that affects your daily life.  This questionnaire is for you to provide more detailed and up to date information about your condition/impairment.

We have recently introduced improvements that will enable you to access the support you need much more quickly and effectively.

It is important that you complete this survey so we have your most up to date information.

If other occupants in your household have a condition/impairment that affects them, please use the same link to complete a questionnaire for them also. If they are under 18, you can provide your own email address as we will only ever contact you as the main tenant.
