Overview

This questionnaire is for you to provide more detailed and up to date information about any conditions or impairments that affect your daily life. It also has a section that enables you to tell us about any adjustments you may require accessing the services in housing the same as everyone else.

We have made improvements to our IT system that will enable our officers to view these adjustments so they can provide any support you need much more quickly and effectively.

It is important that you complete this questionnaire so we have your most up to date information. You can also complete this questionnaire if your situation changes at any time in the future.

If other occupants in your household have a condition/impairment that affects them, please use the same link to complete a questionnaire for them also. If they are under 18, you can provide your own email address as we will only ever contact you as the main tenant.
