We want to hear from parents and carers about their experiences of Havering's Children with Disabilities Short Breaks Service. Your feedback is incredibly important and will help us understand what is working well, what could be improved, and what matters most to families.

The information you share will directly influence how services are planned and delivered in the coming years, helping us provide the best possible support for children and young people with disabilities and their families.

The survey should take around 10 minutes to complete, depending on how much detail you would like to provide. Please answer the questions that are relevant to you and your family's experiences.
